A $250.00 deposit must accompany this application. The deposit is not refundable unless we are unable
to accept the candidate. The balance is due by May 1, 2009. Make checks payable to: TAMARACK
CAMPS. Financial aid is available.
There will be a $100 discount on applications received before December 31, 2009. Age Range: 12
through 16 (younger applicants with exceptional tennis interest will be considered.)

The above fee includes lodging, meals, instruction, laundry, and tennis balls. Purchases made in the
tennis shop or on weekly trips to town will be charged to the camper's account. Early departure from camp
for any reason will not entitle one to a refund.

Clothing list, medical form and information for parents will be mailed upon receipt of application. All
campers are required to have their own health and accident insurance.
